Professional Hormone Balance Evaluation 

Our pharmacists have extensive training in bioidentical hormone therapy and can assist you and your health care provider in determining what therapy is right for you.  We are NOT doctors and we do NOT have doctors on staff.  We work WITH your existing health care provider to make a recommendation for you.

Here's How It Works

1. Complete a Hormone Test Kit (optional but very helpful).
2. Complete our secure evaluation form.
3. We will contact you to set up an appointment to review your information.
4. We will then fax our recommendations to your health care provider for appproval.
5. Once approved, we can fill your prescriptions for bioidentical hormones.

The cost for this service is $45.  To get started:

• Once your order is complete you will receive an email with instructions to access to our secure evaluation form.
• Complete and submit the form and we will contact you to make an appointment to speak with you by phone.
